Created for my employer SPC, this ColdFusion MX 7 application allows any college employee to easily create online surveys and forms with little or no web development know-how. Results from submissions are aggregated and available to the survey creator as reports, graphs, charts, and Excel spreadsheet downloads.
Survey/Form Features
- Build your survey and then simply link to it from your website or email your link to people
- Add images and Flash movies to surveys and their questions
- Specify date ranges for your survey to be open
- Customize your survey/form by uploading your own jpg image for the top banner
- Choose whether or not to have submission confirmations emailed to the user

- Require employees or students to log in with their username & password for access to your survey
- Prevent the same person from submitting twice (based on name, student ID, ssn, or email address)
- Password protect your survey/form with a 4 character pincode
